Columbia Community Care takes an active role in Medicare Fraud, Waste and Abuse prevention within its Medicare Advantage Program and is required by federal regulations to respond promptly to fraudulent conduct.

You may report Fraud, Waste and Abuse through one of the following anonymous methods. Anonymity will be retained to the extent that it does not interfere with investigation results.
